Discount application form asking control code repeatedly

I’m working for Education department and would like to apply for discount for government organization. I put all necessary information on together with 3 letter control code and press “send”. But the page asking the control code repeatedly (with different code) and does not allow me to complete the request.

What’s wrong and what should I do?

And if completed, how can I apply this with on-line purchase?

Please advise.
