I think I have a virus again but avast is no help

I’m having the same problem where I sign on to my ISDN internet connection and then it runs way too slow and the sent packets is always about twice the received packets. I’ve run avast! and Spybot search & destroy and AdAware personal to no avail. Spybot search and destroy always comes up with the same DSO Exploit registry changes every time I’ve ever run it and they won’t go away no matter how many times I “Fix selected problems” but that has been that way forever. What I’m particularly suspicious about are these two processes:
C:\WINDOWS\system32\defragfatx.exe
C:\WINDOWS\system32\windns.exe
In fact the only way I’m on line right now is by stopping every process that doesn’t make my computer crash and reconnecting to the internet. Also, just before the trouble avast! started coming up with messages that said something like ‘connection timed out’ and mentioned windns.exe and asked if it should keeping waiting or not.

Anyway, here is my most recent hijackthis log taken while connected to the internet. Please help and should I get some sort of firewall to prevent this nonsense, any recommendations? Thanks,
