Incredibar has got to go....but how?

I have no clue what I did to get Mystart Incredibar installed. I have spent a huge amount of time on Google searching for similar problems. I have also searched this forum. All advice has been followed, ZERO success. It is driving me crazy.
Most sites suggest Malwarebytes. I downloaded, got the current updates, nothing found.
All sites said to go to program uninstall and remove it. I had 4 programs all installed the same day/time and removed them all. No luck. Still there.
Chrome is my default browser. Some said to go to extensions and delete Incredibar there. I did. Still there.
I went to IE which I never use. Some said to go to settings, search engines and ‘remove’ Incredibar. Remove is grayed out - not an option. I can remove Google and Bing, but I cannot remove Incredibar.

In sort, my searches within this forum and throughout the internet via Google have been of little to no help. Hoping someone has a better idea.

Two things.

Please remove your email address in the user field, as this can cause unwanted spam mail to arrive in your inbox.

Run AdwCleaner to get rid of bad toolbars. You can get that here: http://forum.avast.com/index.php?topic=53253.0 If any other issues are now present, suggest running, in addition, these three programs only: Malwarebytes, OTL, aswMBR.exe and attach all these logs in your next reply. See attached picture below: Tick off “Notify me of replies” to take care of any issues of being notified of any replies in a timely manner.
