Could anybody recommend how to ensure the highest level of security settings for IE 8.0?
I already have my settings set to ‘High’. However, the ‘custom level’ offers the option to further customise and increase the security setting as desired.
What are you settings for the looooooooooooooooooooooooong list in custom level? How do you secure IE 8.0?

hpHosts is a community managed and maintained hosts file that allows an additional layer of protection against access to ad, tracking and malicious websites.
If you use just for that, don’t worry too much.
The protection for IE8 is given by keeping it updated at most. The problems are, generally, exploits and they’re corrected by updates (when Microsoft can…).
I’m not answering your questions because I don’t know a manual for that settings.
I’m posting what I do: use IE only when needed and with a good antivirus behind
IE8 is already quite secure (more secure than FF, at least : ). However, since it’s the most used browser, there are far more exploits for it.
ActiveX are downloaded only if you allow it, so you just need some common sense.
As Tech said, the main thing you can do is to keep it updated.
If you have avast! 5 Pro, you can sandbox it, too.
