How do you know they aren’t being inserted? You can’t check in your sent items folder, you would have to send on to yourself.
Are you using rich text .rtf format in your email?
I think that this has been mentioned before as causing a conflict with inserting notes into clean messages.
Found a problem. Seems it was caused by my personal digital signature. It prevented message from changes.
