I assume you have these forms on file for distribution already but have attached blanks of both for reference.
I was instructed by our accounting controller that this is required by the IRS as part of FATCA reporting requirements. I submitted this request last week using your web-based “contact us” form but did not receive a response.

That was the message I submitted via a support ticket back in September. Have not had any response other than the automated email acknowledgement. Thought maybe it should have been a customer service ticket so opened one of those a few weeks later. No response to that either.
I tried using their “contact us for tech support” messaging link but it just comes back “error submitting - try again later”.
My request is not that difficult. Have had good response to the other tickets submitted so not sure why this is an issue…
Anyone have a PDF of one of these tax forms from Avast? They are not personalized, simply a statement from them. Have had no trouble obtaining from Microsoft, Adobe, TeamViewer, Mozy, etc… Failing that, anyone know how to prompt a response from Avast on unanswered tickets?
