Before startup this box appears:
(Before ‘Welcome to Windows’)

The last time this happened all my file associations became corrupt and completely messed up my computer.

This is the second time it has occured but nothing bad has happened yet.

Can anyone shed any light on this?

Virus and spyware scans reveal nothing.

I use Avast Anti Virus and Comodo firewall.

Windows XP is fully up to date.

Can you schedule a boot time scanning with avast?
It will be good if you download, install, update and run AVG Antispyware. Some users recommend SUPERantispyware, Spyware Terminator and/or a-squared (take care about false positives).
