if i update my windows 8.1 to latest update. and if after few months if i reinstall windows os as shown here in screenshot.
would i loose my windows latest updates if i reinstall windows os as shown in screenshot?
Ofcourse you will loose the updates unless you use a installation medium that has them slipstreamed in it.
what i can do so that latest windows update does not gets deleted when i reinstall windows?

Download all updates and slipstream them into a installation medium.
If you use the option you arrowed, yes you will lose everything, including all your personal files. All of them!
