Twice in the past week, all of my passwords in Bank Mode disappeared.
Any suggestions?

I am sorry, but the point of bank mode is to create the most secure environment for using online banking as possible. As part of that, in bank mode browser deletes all of the data after it is turned off by design.
In case you still want to stay logged into the bank, I can still recommend using normal ASB window as the next best alternative.
