  mention the name of your Operating System; if its
  Win XP, also mention if SP1 or SP2 . Worms are best
  dealt with using a program oriented towards them; if you
  have Win XP, I recommend you download, install, update
 "Ewido" from www.ewido.net/en . Or use their
  Online Scanner .

What was the file name, where was it found example (C:\windows\system32\infected-file-name.xxx) ?

If it is appearing every day, something is either restoring it or you continue to visit the same suspect site. If it is the former then ewido (run from safe mode) should help to detect the program restoring it.
