I too get the message when starting Outlook 2003 “The add-in “C:\Program Files\Alwil Software\Avast4\ashOutXt.dll” could not be installed or loaded. This problem may be resolved by using the Detect and Repair on the Help menu.”
Tried Detect and Repair - no good. searched for extend.dat. not there, checked disabled items, not there, checked addin manager, it’s there. uninstalled office and avast, reinstalled office first then avast and the problem persists.
The Detect and Repair you are talking about is I assume in Outlook ?
Try a repair of avast. Add Remove programs, select ‘avast! Anti-Virus,’ click the Change/Remove button and scroll down to Repair, click next and follow. You need to be on-line to do this.

Repair of Avast done with no change in the error.
Any other ideas?
OK, I thought you had only tried a detect and repair only in Outlook.
Does this file exist “C:\Program Files\Alwil Software\Avast4\ashOutXt.dll” ?
Is the plug-in disabled in Outlook ?
Outlook 2003 > Help > About > Disabled items
If the file is present and the plug-in isn’t disabled in Outlook, I’m a bit lost (I don’t use either Vista or Outlook), I would have suggested if the repair failed that you reinstall but you have tried that and no joy.
Didn’t even think to look for it, nope, it’s not there. But it’s also not there in my XP machine???
There are no disabled items in outlook.

P.S. While thinking about my response, I noticed in Vista, un-hiding hidden files does not show all, when doing a search, you must go to advanced search and be sure and check box next to “Include non-indexed, hidden and system files”

Neither of those should have to wait for a sub-system to start, this is commonly seen when waiting for MS Outlook (when started it goes away) to start when you have the Outlook/Exchange provider enabled.
Unless you are using another email client to MS Outlook then you shouldn’t need it enabled.
So now you have un-hidden files do you find the ashOutXt.dll file ?
It is present on my XP Pro and avast Home installation (mine is installed on a different partition, but yours should be there).
I would have thought that a repair should have replaced the missing file all I can suggest is to uninstall, reboot and install avast and se if that missing file appears.
The files were not missing, just had to make sure I searched for hidden files and it found them.
I will check again tonight and see if the Web Shield and internet mail are still waiting for a subsystem to start. But my Outlook 2003 is coming up w/o error messages.
