How do I get the starter edition? Is the free download on the main page the starter edition or a free trial? Do I have to download Google Pack?

Yes, you do have to download Google Pack, but you can choose just the things you want.
At the bottom of the page I gave you the link to there is a link to download Google Pack.
when using the sd starter edition you only get half of the signatures to get all signatures you must buy the full edition.once you install it through the google pack you can uninstall google if you dont want it and it still leaves sd installed.
I don’t know about the full signatures but there is another way to download it. Download PCTOOLS FIREWALL PLUS and there is a link to download and install SD a full functional program but you have to pay if ever you get a nasty and want to remove it through SD. I personnally tried it 3 times and I had to remove it, it is too demanding on Energy (CPU) and conflicts with some other softwares (SpywareBlaster Immunizzation). 
yup only half of the sigs(check there forum to see),thats why i would not use it,i think that giving only half of the sigs is ridiculous,i mean the free version is already limited in the guards that they offer in free version(but thats my opinion)i think sd full version is a very good product even though it is heavy on the pc.
I’ve give up on Spyware Doctor Starter Edition too.
Users deserves respect and protection, I can’t stand on half of the signatures even for a freeware.
